Post Traumatic Facial Nerve Palsy, Facial Nerve Decompression Procedure

A young male patient met with road accident and came to emergency with Right side Immediate onset facial nerve paralysis. It was lower motor neuron type with Severity Grade V.

HRCT TEMPORAL BONE revealed fracture line in relation to vertical part of fallopian canal (bony canal of facial nerve).

Patient was counselled regarding prognosis, chances of recovery and possible complications. Patient also had co existing CSOM (Chronic infection if ear with hole in ear drum).

Mastoid Exploration with Facial Nerve Decompression and Tympanoplasty was performed. Followed by facial physiotherapy and Facial nerve stimulation.

After 6 months, Facial Nerve function improved from Grade V to II/III with complete eye closure achieved.
